Dear Tryout Participant,
We hope you are looking forward to the exciting challenge of college athletics. As part of our screening process, students looking to try out for a University of Hartford athletics team must fill out our (1) student-athlete health history questionnaire, (2) pre-participation physical exam (completed by your physician), (3) medical authorization (parts A-D), and (4) health insurance information/authorization insurance form. The information that you provide will be vital for your future care. It is extremely important for us to know any past injury, illness, and/or problems which you may have sustained. Please print out all of the forms and take the time to fill them out with as much detail as possible, in order to make a smooth transition into Hartford Athletics.
Please fill out the forms, as soon as possible, and attach a copy of your health insurance card (both front and back) and return them to us before the try out date.
Starting in 2010-11, the NCAA has passed new legislation for sickle cell testing. For more information and to fill out the required sickle cell testing form, click here.
